Comprehensive Medication Management (CMM) is described as the standard of care that ensures each patient’s medications are individually assessed to determine if it is appropriate, effective, safe and able to be taken as intended by the patient. It also includes a monitoring plan to ensure that the patient will achieve the intended goals of therapy and follow-up to determine actual outcomes. It is based on an ongoing patient-pharmacist relationship and having a willing and engaged patient is necessary to achieve the desired outcomes. This presentation will also include a review of published articles of successful implementation, physician and patient satisfaction and possible billing codes for its payment.
